Eviction Prevention: Requires a referral from Department of Health and Human Services/Emergency Services and an eviction notice. FINANCIAL ASSISTANCE: Offers financial assistance for eviction prevention, utility shut-off, and prescriptions. Assistance available once every 12 months per eligible client. Eligibility Anyone in crisis in the service area. Availability May be used at most 1 time per 12 month(s)Application Process Referrals through DHHS/Emergency Services. Accepts self referrals for prescriptions only with required documentationDocuments Required Proof of address, picture ID, referral, proof of income, and proof of need.
